The table below outlines the requirements in applying for financial aid. Please read carefully including additional details below. In order to prevent delays in the disbursement of financial aid, please complete all the requirements outlined below.

Item #1 – Free Application for Federal Student Aid (FAFSA)
The FAFSA is the first step in the financial aid process and is used to apply for student financial aid such as grants, loans, and scholarships.
ITEMS NEEDED FOR FAFSA COMPLETION:
- Your Federal Student Aid account username and password (FSA ID)
- Note: This may take time to process, so it’s recommended that you create your account 4 days prior to filing the FAFSA. Visit studentaid.gov/fsa-id to create your account.
- Your Social Security Number
- Name, date of birth, email address, and social security number of each contributor to the FAFSA
- Your 2022 federal income tax return 1040 and accompanying schedules
- Note: If completing the FAFSA online, you will automatically retrieve your income and tax data from the IRS and have it transferred into your FAFSA.
- Your current bank statements and records of stocks, child support payments, bonds, 529 plans, business and farm assets, and other investments including real estate (but not including the home in which you live)
- A personal email address (not a school email address)
HELPFUL TIPS FOR FAFSA COMPLETION:
Student's Grade Level in College
- First Year Conemaugh Students: please select Never Attended/1st Year OR Attended College Before/1st Year
Type of Degree / Certificate
- Nursing Students please select Certificate/Diploma (occupational/technical/education program of at least 2 years)
- Rad Tech Students please select Associate Degree

Item #2 – PA State Grant Application (School of Nursing Only)
The PA State Grant Form can be completed with the FAFSA by clicking the link that reads “Optional Feature – Start your state application – Click here if you want to apply for Pennsylvania state-based financial aid” on your FAFSA confirmation page. Otherwise, you must allow 48 hours after FAFSA completion before completing a PHEAA state grant application on www.pheaa.org.
Item #3 – Master Promissory Note (MPN) Loan Agreement
The Master Promissory Note is the application and loan agreement for the Federal Direct Subsidized and Unsubsidized Student Loans. The MPN should be completed with the entrance counseling at https://studentaid.gov/h/complete-aid-process. Please select CONEMAUGH MEMORIAL MEDICAL CENTER. An MPN must be completed to be eligible for a Federal Direct Student Loan.

Item #4 – Direct Loan Entrance Counseling
Direct Loan Entrance Counseling is required in order to borrow a student loan and should be completed along with the Master Promissory Note on https://studentaid.gov/h/complete-aid-process. This counseling session is informational only so any numbers and amounts used are not actual.
