Clinical Pharmacist Specialist – Critical Care

Dr. Formeck serves as a preceptor for the critical care rotation. She earned her Doctor of Pharmacy degree from the LECOM Erie School of Pharmacy and completed a PGY1 Pharmacy Practice Residency at Conemaugh Memorial Medical Center.  After completing her residency, Samantha stayed on as a staff pharmacist for 3 years prior to transitioning to her current role as a clinical pharmacist in Critical Care.  She earned her board certification in critical care pharmacy in 2022.  She values the opportunity to precept students and residents to help shape the future generation of pharmacists.
