Donor Recognition Kiosk

An interactive kiosk is featured with Conemaugh Health System's Donor Recognition Wall at Conemaugh Memorial Medical Center. The kiosk offers a method for continuous appreciation of local organ, tissue, and eye donors and the selfless donations that saved others.

The kiosk features living donors, deceased donors, and recipients including donor portraits and information about his or her gift of life.

Deceased Donors
Each year, members of Conemaugh Health System's Donor Advisory Committee contact a deceased donor's next of kin with information to participate in the Donor Recognition Wall and Interactive Kiosk. If a loved one donated and were not contacted by the Donor Advisory Committee, contact Conemaugh Health System Marketing Communications at (814) 534-9611.

Living Donors
To share a story of a local living donor, complete the Living Donor Kiosk Participation Form and Disclosure Authorization Form. Send completed forms and photo of donor to the contact information listed on the participation form.

Recipients
To share a story of a local organ, tissue, or eye recipient, complete the Recipient Kiosk Participation Form and Disclosure Authorization Form. Send completed forms and photo of recipient to the contact information listed on the participation form.
