Notice of Patient Privacy Pledge
At Conemaugh Health System, patient privacy is a priority. Each time a patient visits one of the facilities within Conemaugh Health System, a record is created for the visit. Generally, the record contains information including symptoms, examination and test results, diagnosis, plan for future care and treatment, and billing-related information. This record is needed to provide each patient with quality care and to comply with legal requirements.
The Conemaugh Health System Notice of Patient Privacy Pledge applies to all records of care generated by any of the facilities within Conemaugh Health System, whether made by Conemaugh Health System personnel, physicians, or other individuals authorized to access or document in a patient's health record. Each physician may have additional policies or notices regarding the use and disclosure of health information.

Notice Regarding Health Information Exchanges
For your ongoing health care treatment purposes, we may share your information using a variety of Health Information Exchanges and networks both on a regional and a national basis. You have the right not to participate in these exchanges. If you choose not to participate in the exchanges, your health information will no longer be accessible through the exchange. However, it does not affect the information that was exchanged prior to the time you chose not to participate.
